By providing your telephone number(s) and confirming your acceptance of SMS/text messages from Modern Chiropractic and Pain Relief, PC through electronic and/or digital means, you expressly consent to receive recurring non-marketing and marketing text messages from Modern Chiropractic and Pain Relief, PC. Messages will originate solely from Modern Chiropractic and Pain Relief, PC. You may opt out of these communications at any time. Message and data rates may apply. Message frequency varies.

Program Description

Modern Chiropractic and Pain Relief, PC and its service providers may use an automatic telephone dialing system or automated text service to deliver text messages to you. Modern Chiropractic and Pain Relief, PC text messages are intended to provide you with information about our healthcare services, appointment reminders, billing updates, and promotional communications.

Cost

Message and data rates may apply to each text message sent or received in connection with Modern Chiropractic and Pain Relief, PC text messages as provided in your mobile telephone service rate plan, in addition to any applicable roaming charges. Please contact your mobile telephone carrier for pricing plans. Modern Chiropractic and Pain Relief, PC does not impose a separate fee for sending text messages.

How to Opt In

To opt in to receive text messages from Modern Chiropractic and Pain Relief, PC, please follow the instructions provided when you enroll via our website, patient intake portals, digital appointment requests, or clinical consent forms.

How to Opt Out

To stop receiving text messages from Modern Chiropractic and Pain Relief, PC, reply “STOP” in response to any text message received from our text messaging program. You will then receive a single confirmation message of your opt-out. Reply “HELP” for customer support or assistance.

Your Mobile Telephone Number

You represent that you are the account holder for the mobile telephone number(s) that you provide. We are able to deliver messages to major mobile phone carriers. Carriers are not liable for delayed or undelivered messages.

Access or Delivery to Mobile Network Is Not Guaranteed

Delivery of information and content to a mobile device may fail due to a variety of circumstances or conditions outside our control. You understand and acknowledge that mobile network services are outside of the control of Modern Chiropractic and Pain Relief, PC, and we are not responsible or liable for issues arising from network carrier delays or delivery failures.

Eligibility

To receive Modern Chiropractic and Pain Relief, PC text messages, you must be a resident of the United States and 18 years of age or older. Modern Chiropractic and Pain Relief, PC reserves the right to require you to prove that you are at least 18 years of age.

Changes to Terms and Conditions

Modern Chiropractic and Pain Relief, PC may revise, modify, or amend these SMS Terms and Conditions at any time. We will provide notice of any such revision through reasonable means, including by email to an address you provide or by text message to a telephone number on file. Modifications will take effect seven (7) days after notice is provided. If you do not agree to the modified terms, you must opt out within the 7-day period.

Termination of Text Messaging

Modern Chiropractic and Pain Relief, PC may suspend or terminate your receipt of text messages for any reason, including if we believe you are in breach of these SMS Terms and Conditions or if your mobile service lapses. We reserve the right to modify or discontinue text messaging services, temporarily or permanently, with or without notice.
